  1. If taking multiple programs (Combination eduVacations in different destinations), please send separate forms for each destination. You can exclude the address field down to the emergency contact on additional forms; just make sure you input your Name.
  2. You can submit the form on-line, or mail it to eduVacations (1431 21st Street NW Suite 302, Washington, DC 20036-6930) or fax it to 202-463-8091.
  3. If more than one eduVacator is in your party, please complete a form for each participant.
